Summary:
Functions as part of an interdisciplinary team of physicians, nurses and other professionals to maintain continuity of patient care for all patient populations from admission to discharge. Utilizes appropriate criteria for inpatient care and plans timely discharges. Provides emotional support to patients and families. Actively participates in designing and implementing institutional program/initiatives to improve patient access to the medical center resources, quality of care, cost reduction efforts and patient satisfaction.
JOB REQUIREMENTS:
Master''s of Social Work degree from an accredited college or university
One (1) to two (2) years effective experience
PREFERRED
Qualifications: Hospital-based social work experience.
