Clinical Pharmacist - Oncology at Penn State Health in Hershey, PA

SUMMARY OF POSITION:
Responsible for providing the prospective evaluation and monitoring of drug therapy and make patient specific recommendations to improve pharmacotherapy outcomes for specialty clinic patients and outpatient groups. Provide education regarding drug therapy to health care professionals, patients and caregivers as well as instruct and educate pharmacy students and pharmacy residents.
Essential Functions:
The percentage of time spent performing essential functions is 95%.Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Perform all of the functions of an Outpatient Specialty Pharmacist. Participate in the multidisciplinary care of specialty clinic patients with physicians/prescribers and other health care professionals. Monitor patient therapy including side effects, laboratory values, patient outcomes, medication reconciliation, and answering of drug information questions initiated by the medical team. Optimize therapy, decreasing risk of adverse effects, and improve cost by recommending appropriate changes in aspects of medication therapy including choice of drug, dose, frequency, and duration. Provide education to patients/caregivers regarding medication therapy. Provide clinical monitoring of patients while meeting accreditation measures.Assist with prior authorization, appeals and peer to peer processes for your provider team. Work with your provider teams to increase medication adherence and positive outcomes.Decrease adverse events and side effects by providing regular monitoring and assessments with patients via telephone or in clinic. Instruct and educate medical and pharmacy residents and students, including precepting of pharmacy students and residents. Assure compliance with Drug Use guidelines by monitoring patient therapy and assessment of appropriate guidelines Collect and analyze data specific to patient therapeutic category. Participate in accreditation and program growth initiatives. Review and interpret medical literature and apply conclusions to specific patient situations to develop the most practical and evidence-based recommendation to optimize drug therapy. MINIMUM
Qualifications:
Bachelors of Science degree in Pharmacy or Doctorate Degree in Pharmacy required. ASHP-Accredited PGY-1 Pharmacy residency or 3 years of relevant clinical pharmacy experience required. Active and valid PA licensure or eligibility as a Pharmacist required. KNOWLEDGE, SKILLS, & ABILITIES:
Ability to prioritize and organize daily activities to meet patient care needs required. Effective verbal communication skills, time management skills and ability to manage multiple priorities required. Ability to analyze clinical data and perform calculations. Computer and Microsoft Office skills required. Extensive knowledge of pharmacotherapy required. Attention to detail and critical thinking skills required. Effective mentoring and precepting abilities required. PREFERRED
Qualifications:
Clinical experience in either Oncology, Rheumatology, or Dermatology
Salary Range:
$100K -- $150K
Minimum Qualification
PharmacyEstimated Salary: $20 to $28 per hour based on qualifications.
